What do we do?

Môn Community Link is a social prescribing service open to anyone over the age of 18 living on Anglesey who may be feeling isolated or disconnected from their community. Our team of Local Asset Co-ordinators will work with you to identify activities and community services in your area that may be of interest, offer support for you to access these activities and services, and help to improve your wellbeing, confidence and independence. The project is part of Medrwn Môn, the County Voluntary Council for Anglesey.

What information do we collect?

We collect personal information provided to us on the referral form from your GP surgery and from other partners - including Isle of Anglesey County Council, Betsi Cadwaladr University Health Board, North Wales Police and community organisations – as well as information we gather from you. This includes contact details and may include detailed information about your personal circumstances.                                                                                 

Why do we collect information?

We do this so that we can keep in touch with you, provide appropriate support and report on our project.                                                                                                                                        

How do we store the information we collect?                                                        

We will sometimes store information on paper when we make notes during our meetings with you. We transfer these notes on to our secure, online, management system and securely destroy the paper copies once they are on the system. We ensure that all your data is stored securely.

Who do we share it with?                                                                                                  

Only staff with explicit reason to do so have access to your data. We share anonymous summary information with our funders for reporting and research purposes. We only share your personal information with your GP surgery to inform them of progress if requested and/or if there are any safeguarding concerns. Where necessary and lawful, or when required by legal obligation, we may also share information with partners including:

• contractors, associates or representatives of the person whose personal data we are processing,
• next of kin,
• healthcare, social and welfare organisations

We will also share your information with any organisation that you ask us to in order to support you.

How long do we hold your information?                                                                  

We will keep your personal information for 15 months after our last contact with you. It will then be securely destroyed.

How can you access or correct the information we hold about you?
